Keeping our students healthy.

Each year it is important to make sure that all of your student's health records are updated.  This way our School Health Office can maintain an excellent standard of care.

LTCS School Health Program

The School Health Program at LTCS advances the well-being, academic success, and life-long achievement of students. To that end, our school health specialist facilitates positive student responses to normal development; promotes health and safety; intervenes with actual and potential health problems; provides case management adaptation, self management, self advocacy, and learning.

The School Health Specialist:
Provides health services to students 
Provides screening and referral for health conditions
Promotes a healthy school environment
Promotes health
Serves in a leadership role for health policies and programs
Serves as a liaison between school personnel, family, community, and health care providers

Healthy children are successful learners. The school health specialist has a multi-faceted role within the school setting, one that supports the physical, mental, emotional, and social health of students and their success in the learning process.

Meds Administration

Any students requiring specific health related plans should have a Student Health Plan on file with the School Health Office.  This form may be found at the link below.

The School Health Office is not allowed to administer any medicines without a Medication Authorization Form on file.  This includes over the counter medications, prescription medications, and throat/cough lozenges.  The Medication Authorization Form can be found by clicking on the link below.

In addition to medication administration, some students may require specific plans for a variety of health conditions, including diabetes, asthma, seizures, and self-carry medications.  In order for our School Health Office to provide the best care, please make sure that the appropriate plan has been submitted for your students.  Each plan form can be found below.

Health Records

Updated health records must be maintained for all students.  These health records include immunization records (or religious waiver form) that must be submitted prior to the start of kindergarten and again prior to the start of seventh grade.  Also included in these mandatory health records is a Hearing and Vision Form 3300.  The immunization record and form 3300 both may be obtained through your pediatrician.  The religious waiver of immunization can be found at the link below.

Water Safety

HB 402 requires Georgia school districts to provide water safety education annually to parents and guardians of students under 18 years of age and directly to students who are 18 years of age or older.  
 
Water safety education resources are provided by the Red Cross and the YMCA.  See attachments below:
