Meds AdministrationAny students requiring specific health related plans should have a Student Health Plan on file with the School Health Office. This form may be found at the link below.
The School Health Office is not allowed to administer any medicines without a Medication Authorization Form on file. This includes over the counter medications, prescription medications, and throat/cough lozenges. The Medication Authorization Form can be found by clicking on the link below. In addition to medication administration, some students may require specific plans for a variety of health conditions, including diabetes, asthma, seizures, and self-carry medications. In order for our School Health Office to provide the best care, please make sure that the appropriate plan has been submitted for your students. Each plan form can be found below. Health RecordsUpdated health records must be maintained for all students. These health records include immunization records (or religious waiver form) that must be submitted prior to the start of kindergarten and again prior to the start of seventh grade. Also included in these mandatory health records is a Hearing and Vision Form 3300. The immunization record and form 3300 both may be obtained through your pediatrician. Water Safety
HB 402 requires Georgia school districts to provide water safety education annually to parents and guardians of students under 18 years of age and directly to students who are 18 years of age or older.
